Change of collection

Authorisation for collecting children at the end of the school day.

We will not allow children to be collected by adults who are not known to us or authorised by you.

Please note that we may need to telephone parents to confirm the identity of anyone who is unfamiliar.

ALL BOXES must be completed when you update this form – place an X or N/A in any empty boxes.
